目的 比較頸前小切口入路甲狀腺手術(shù)及改良Miccoli腔鏡輔助下甲狀腺手術(shù)治療甲狀腺良性腫瘤的療效。
方法 回顧性分析筆者所在醫(yī)院2010年1月至2012年1月期間收治的80例甲狀腺良性腫瘤患者的臨床資料,按手術(shù)方式將其分為頸前小切口組(n=40)和改良Miccoli組(n=40),比較2組患者的手術(shù)時間、術(shù)后頸部引流量、美容滿意度、住院時間、住院費用及并發(fā)癥發(fā)生率。
結(jié)果 與改良Miccoli組比較,頸前小切口組患者的手術(shù)時間較長〔(95.5±20.3) min比(62.4±15.5) min,P<0.05〕, 而美容滿意度評分〔(3.5±0.2)分比(4.3±0.1)分〕和住院費用〔(5 814.6±1 262.3)元比(9 846.7±1 080.5)元〕均較低(P<0.05); 2組患者的術(shù)后頸部引流量〔(28.6±5.5) mL比(22.2±4.5) mL〕和住院時間〔(4.5±1.5) d比(3.8±0.9) d〕 比較差異均無統(tǒng)計學意義(P>0.05)。頸前小切口組有3例患者術(shù)后出現(xiàn)聲音嘶啞,均于術(shù)后1個月左右恢復; 改良Miccoli組有1例患者術(shù)后出現(xiàn)聲音嘶啞,于術(shù)后3個月左右恢復。2組患者的喉返神經(jīng)損傷發(fā)生率比較差異無統(tǒng)計學意義(P=0.608)。2組患者均無其他手術(shù)并發(fā)癥發(fā)生。
結(jié)論 改良Miccoli腔鏡輔助下甲狀腺手術(shù)的頸部美容效果優(yōu)于頸前小切口入路手術(shù),但前者的住院費用高于后者,對美容有特殊要求或是從事特殊職業(yè)的患者可選擇性使用改良Miccoli腔鏡輔助下甲狀腺手術(shù)。
