• 解放軍第一四八中心醫(yī)院介入血管科（山東淄博 255300）;

目的 　探討機械性抽吸聯(lián)合支架植入治療急性髂、股靜脈血栓和閉塞的臨床效果及應用價值。
方法　 筆者所在醫(yī)院自2004年10月至2011年12月期間，收治了273例急性髂、股靜脈血栓合并髂靜脈狹窄或閉塞患者，左側235例，右側38例。局麻下經(jīng)皮穿刺患側股靜脈入路，對髂、股靜脈血栓，在導絲引導下插入8～14F鞘管行機械性抽吸，其中53例插入18～24F特大鞘管行機械性抽吸大塊機化性血栓。對股、腘靜脈血栓，經(jīng)健側股靜脈在導絲抓捕技術輔助下，置入翻山鞘管至患側，在鞘管支撐下導絲輕柔地越過瓣膜插至患側腘靜脈處，沿導絲置入Fogarty球囊導管，在數(shù)字減影血管造影（DSA）透視下順應靜脈瓣膜方向將血栓拉至患側髂靜脈內，再經(jīng)患側股靜脈進行機械性抽吸血栓。在髂靜脈血栓機械性清除后，對造影顯示的血管狹窄或閉塞段，再進行經(jīng)皮血管腔內成形術（percutaneous transluminal angioplasty，PTA）和支架植入。
結果 　血栓完全清除（Ⅲ級）219例（80.22%），部分清除（Ⅱ級）49例（17.95%），清除最少（Ⅰ級）5例（1.83%）。支架植入后，治愈235例（86.08%），顯效29例（10.62%），好轉7例（2.57%），無效2例（0.73%）?？傆行蕿?9.27%（271/273），平均住院時間7.5d，患肢腫脹和疼痛均于介入治療后1～2d內開始消退或減輕。隨訪療效：3～6個月血流通暢259例（94.87%）；7～12個月血流通暢254例（93.73%），13～24個月血流通暢236例（87.08%），25～36個月血流通暢228例 （84.13%）。
結論 　機械性抽吸聯(lián)合支架植入治療急性深靜脈血栓形成和髂靜脈閉塞，療效顯著、病程短，是安全有效的方法。
