目的 對尿液特征組分與糖尿病早期腎損害的關(guān)系進(jìn)行了初步探索。 方法 對2011年12月-2012年5月間28例2型糖尿病組、33例2型糖尿病腎病組及26例健康對照組尿液中尿蛋白含量和幾種常見非蛋白氮物質(zhì),包括肌酸、尿囊素、肌酐、尿酸和假尿嘧啶核苷的濃度進(jìn)行測定,采用多種歸一化方法對數(shù)據(jù)進(jìn)行對比分析,并通過t檢驗(yàn)減少高效液相色譜測定的變量信息,保留P<0.05的檢出峰進(jìn)行主成分分析,獲得分類結(jié)果。 結(jié)果 采用體積歸一化方法,發(fā)現(xiàn)健康對照組尿液中肌酸、尿囊素和尿酸的含量與2型糖尿病組和糖尿病腎病組相比,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05),2型糖尿病組尿液中尿蛋白的濃度與糖尿病腎病組相比,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。 結(jié)論 通過肌酸、尿囊素、尿酸和尿蛋白的聯(lián)合測定為腎臟損傷程度的監(jiān)測及療效觀察提供依據(jù),為2型糖尿病患者腎功能損壞的早期預(yù)防與診斷進(jìn)行初步判斷提供了新的方法。
